According to the letter, DuPont failed to evaluate a sampling tool used in the validation for Percocet and Endocet tablets that failed blend uniformity.
The letter stated that the last tablets of the compression run in one of three validation lots of Endocet (hydrocodone/acetaminophen) tablets were OOS for compression and that the firm concluded "that the blend was not uniform.
