Fault tree analysis (FTA) is better suited for "top-down" analysis.
Ure's prior novel, "The Fault Tree," which I loved.
Also, the fault tree analysis process used on the health hazard evaluation for the LP 1000 to quantify mitigation factors used in risk analysis was subjective without established numerical values.
