11,21,22,24 To minimise cord infarction during open thoracotomy, segmental arteries should be divided close to their origins to enable collateral circulation through the internal mammary and intercostal arteries.
In both of these patients, the bleeding was coming from the chest wall, with the intercostal arteries being the most likely source.
The eighth and ninth intercostal arteries were found to be lacerated at their origins; their aortic ostia were closed and the distal arteries were ligated.
