PBC is characterised by a T-cell mediated attack on the small intralobular bile ducts.
High-resolution CT showed extensive interlobular and intralobular interstitial thickening superimposed on diffuse ground-glass opacities (the "crazy-paving" pattern), primarily in volving the middle and lower lungs (Figure 2, A through C).
Initially, the degree of intralobular line formation (the fine reticular pattern) is minimal, but this becomes more prominent, and the degree of architectural distortion and traction bronchiolectasis increases with the duration of the disease.
