Possible differential diagnoses to consider are appendicitis, ovarian torsion, phlegmon or hematoma of broad ligament, pelvic or abdominal abscess, urolithiasis, and pelvic cellulitis.
Inflamed diverticulum, phlegmon formation (Hinchey la), and small ([less than or equal to]3 cm) pericolic abscess formation (partial Hinchey lb) were considered to be consistent with the diagnosis of CT-based uncomplicated diverticulitis.
(1) (The Hinchey classification is based on radiologic findings, with 0 for clinical diverticulitis only, la for confined pericolic inflammation or phlegmon, and lb for pericolic or mesocolic abscess.6) Exclusion criteria included suspicion of colonic cancer by CT or ultrasound (US), previous CT/US-proven diverticulitis, sepsis, pregnancy, or antibiotic use in the previous 4 weeks.
Other high suspicion scenarios include prolonged fever with unexplained/culture-negative shock, or antibiotic treatment failure for cervical adenitis or retro/parapharyngeal phlegmon. Consultation with or referral to a regional KD expert may be needed.
The CT findings of appendicitis include dilatation of appendix (>6 mm in diameter), heterogeneity of periappendiceal and pericecal fat as a sign of inflammation, and intraluminal appendicolith and formation of phlegmon or abscess (1) (Figure 1).
In accordance with our results are findings from Bekdas and Ozturk, who, in different clinical setting but also related to infection, assessed diagnostic accuracy of PLR regarding acute complicated appendicitis (defined as a the presence of phlegmon, abscess, or perforation) in pediatric population [22].
