The definition of the severe OHSS form is based on the criteria of Golan and associates (1989) and Navot and associates (1992): pain in the abdomen, ovaries greater than 5cm and ascites or
hydrothorax.
On the one hand, right ventricular heart failure, most often induced by respiratory tract diseases, pulmonary artery stenosis or pulmonary hypertension, induced in turn by left ventricular heart failure (Dunn et al., 1973; Kisloff & Schaffer, 1976), can be a cause of ascites,
hydrothorax, hydropericardium, congestive oedema of parenchymal organs, resulting in the impairment of their function (Birgens et al., 1978; Field & Meyer, 1978; Lambert, 1991).
The patient had a consistent fever 1 day after surgery due to lung infection and
hydrothorax, and he requested to be discharged home 25 days after the surgery without palliative chemotherapy, radiotherapy, and a routine CT follow-up because of the poor prognosis of the disease and financial constraints.

hydrothorax and ascites.

Patients who had history of spinal disc prolapse, previous open renal surgery, and post-operative complication like
hydrothorax that required intervention, pneumothorax, massive haemorrhage that required more than 1 U transfusion and conversion to open procedure were excluded.
Thoracostomy is done frequently in cases of chest trauma, pneumothorax, and
hydrothorax or after cardiothoracic surgeries to drain the collected fluid, blood, or air.
The severe form is described by the presence of large ovarian cysts (>12x12 cm), clinical ascites with or without
hydrothorax, hyperkalemia (potassium >5 mmol/L), hyponatremia (sodium <135 mmol/L), hypo-osmolarity (osmolarity <282 mOsm/kg), hypoproteinemia (serum albumin <35 g/L), oliguria (<300 mL/d or <30 mL/h), creatinine 1.1-1.5 mg/dL, and hypovolemic shock.
