The differential diagnosis includes bowel ischemia, bowel perforation, necrotizing enterocolitis (given the presence of pneumatosis), intra-abdominal abscess, adynamic
ileus, or Ogilvie syndrome.

Patients with burns on > 20% of their total body surface area (TBSA) are at risk for gastric distension and associated nausea and vomiting, which may cause aspiration and
ileus, so a nasogastric (NG) tube may be ordered.
