PUV is the most common cause of severe obstructive
uropathy in children, affecting 1 in 5 000 - 8 000 boys.
This recommendation notes that VCUG should not be used routinely after a first febrile UTI, but should be reserved for use if RBUS reveals hydronephrosis, scarfing, or other findings suggestive of high-grade VUR or obstructive
uropathy. It may also be used in "other atypical or complex clinical circumstances." Level B evidence supports this "recommendation."
The care of patients with obstructive
uropathy is complex.
In the mildest cases and in the most severe forms of fetal
uropathy, sonography alone is a good predictor of outcome, and invasive approaches based on fetal blood or fetal urine biochemistry are therefore unnecessary.
