May-Thurner syndrome (MTS) is a rare disease which occurs due to the compression of the left iliac vein by the overriding right iliac artery, subsequently resulting in obstructed venous return, and ultimately predisposing the patient to deep venous thrombosis.1 Its incidence is variable and ranges between 5% to 18% in patients with DVT involving lower limb.
The pathophysiological mechanism of CVI and VVs consists mainly in alteration of venous return, triggered by the incompetence of venous valves or by an intrinsic defect of the vein wall.
At surgery atrial septation has to be done in such a manner that, systemic venous return is redirected to the right atrium and pulmonary venous drainage is retained in the left atrium.
This causes increasing venous return to the right atrium where it enters the foramen ovale and aorta.
VTE may develop in postoperative patients for various reasons: patient characteristics (advanced age, comorbid disease, for example), inactivity following the operation, and insufficient venous return. VTE prevalence among the postoperative patients was 5.6/10,000 in the present study.
The basic abnormality is a vascular aberration in which the splanchnic venous return drains directly into the systemic veins (IVC or the left renal, left iliac or left hepatic vein), diverting the mesenteric circulation away from the liver.
