Presence of air in the biliary ducts can also be found after ERCP (endoscopic retrograde cholangiopancreatography) interventions or in biliodigestive anastomosis--in these cases, when the patient is examined (through either ultrasound, CT or MRI) the air is present mostly in the left hepatic lobe due to the anatomic disposition of the left biliary ducts (antero-posterior/vertical) in comparison with the right biliary ducts (lateromedial/horizontal); therefore when air is detected in the right biliary tree, angiocholitis must be strongly considered even without a thickened duct wall.
