Histopathology of the parotid mass revealed that it was made up of a group of lymph nodes whose architecture was preserved; the mass contained a proliferation of histiocytes, hyperplastic follicles, enlarged germinal centers, and epithelioid cells with abundant, pale eosinophilic cytoplasm (figure 2). The numerous epithelioid granulomas contained Langhans cells and foreign body giant cells, while the necrotizing lesions contained neutrophilic and eosinophilic infiltrates. A transbronchial lung biopsy (Figure 2) showed interstitial chronic inflammation and collections of epithelioid cells suggestive of granulomas with negative stains for acid-fast bacilli and fungus and, on review, diagnostic of hypersensitivity pneumonitis. |
