If there are any concerns that the term newborn is becoming hypoglycaemic the infant should be given another breastfeed (Grade A) Individuals with Type A insulin resistance who develop frank diabetes require large doses (>200 units/day) of subcutaneous insulin, oral hypoglycaemic agents and insulin sensitizers and despite this intense regimen glycemic control remains poor and these patients are at high risk of the complications of diabetes, such as cardiovascular disease, nephropathy, retinopathy and neuropathy. Replacing pancreatic islet cells is the only currently available treatment for severe hypoglycaemic episodes that cannot be managed using conventional therapy and is the reason the Tom Mandel Islet Transplant program was established in Melbourne. |
