Lupus anticoagulant is a term used to describe a phenomenon in aPTT testing that occurs when a patient's polyclonal antibodies influence the phospholipid-dependent reaction. Researchers have now identified two closely related lupus autoantibodies, anticardiolipin antibody and lupus anticoagulant (together called the antiphospholipid antibodies), that are associated with risk of miscarriage. Features of APS include hemolytic anemia, thrombocytopenia, venous and arterial occlusions, livedo reticularis, pulmonary manifestations, recurrent fetal loss, neurologic manifestations (stroke, transverse myelitis, Guillain-Barre syndrome); and a positive Coombs test, anticardiolipin antibodies, or lupus anticoagulant activity (1). |
