The RPR test result was nonreactive while the FTA-ABS test result was reactive.
Historically, a "shotgun" approach was used, involving computed tomography (CT) and magnetic resonance imaging (MRI), an electrocardiogram (EKG), urinalysis, renal ultrasound, complete blood count, chemistries, a thyroid-stimulating hormone test, an erythrocyte sedimentation rate, a TORCH profile, and an FTA-ABS test for syphilis, among others.
Like the TPI, the FTA-ABS test is relatively subjective and labor intensive and has largely been replaced by the particle agglutination assay.
Therefore, a positive syphilis IgG or FTA-Abs test (both treponemal tests) is not conclusive of active syphilis infection; the RPR (non-treponemal test) on the infant must be positive as well.(3) The tests performed on the infant should be the same as those performed on the mother to enable comparison of titer results.
Traditionally, the FTA-ABS test has been considered the gold standard treponemal test and still is used by some laboratories.
The CSF FTA-ABS test is less specific for neurosyphilis than the CSF-VDRL but is highly sensitive; neurosyphilis is highly unlikely with a negative CSF FTA-ABS test."
Overall, 211 (24.3%) of sera obtained from these patients were reactive in both the RPR and FTA-ABS tests, a further 236 (27.2%) were reactive in the FTA-ABS test alone, and 421 (48.5%) were negative for both.
The RPR was nonreactive; an FTA-ABS test was not ordered.
