Bisphosphonates, which include alendronate (Fosamax), risedronate (Actonel), zoledronic acid (Reclast), and ibandronate (Boniva) are recommended as the initial treatment.
The approval is based on data from a Phase three study that indicated patients on glucocorticoid therapy who received Prolia had greater gains in bone mineral density (BMD) compared to those who received active comparator (risedronate).
This US FDA approval is supported by the 12-month primary analysis of a 24-month Phase 3, randomised, double-blind, double-dummy, active-controlled study evaluating the safety and efficacy of Prolia 60 mg subcutaneously every six months compared with oral risedronate 5 mg once daily in 795 patients receiving glucocorticoid treatment, greater than or equal to 7.5 mg/day oral prednisone (or equivalent), added the company.

DENVER--The anabolic agent teriparatide reduced the risk of clinical vertebral fractures in women with osteoporosis when compared with the antiresorptive agent risedronate, and the results held up across groups of women with varying fracture histories in a post hoc analysis of data from the VERO trial.
This guideline update focuses on treatment with bisphosphonates (alendronate, risedronate, ibandronate, zoledronic acid) and denosumab.
Unfortunately, osteoporosis is usually under-diagnosed and undertreated in elderly hip fracture patients.14 In recent years, bisphosphonates are widely used for treatment of osteoporosis, such as Alendronate, Risedronate and Zoledronic et al., as those agents have been approved to reduce the incidences of vertebral and hip fracture in elderly patients.15-17 However, whether the bisphosphonates can reduce the risk of second hip fracture is unknown.
Currently, drugs for the prevention and treatment of osteoporosis are mainly categorized into bone resorption inhibitor which includes diphosphonate (alendronate, etidronate and risedronate), sex hormone, calcitonin, selective estrogen receptor modulator and bone formation promoter, fluoride product, vitamin D and calcium preparation, parathyroid hormone included.
As outcomes for men who experience an osteoporotic fracture are poor, treatments that have been shown to improve bone mineral density (BMD) and decrease fractures in this patient population, such as a bisphosphonate (alendronate, risedronate or zoledronic acid) or denosumab, are recommended.
Fifty-three men treated with antiepileptic drugs for at least two years prior to enrollment in the Anti-Epileptic Drug and Osteoporosis Prevention Trial (ADOPT) were randomized to receive risedronate (a drug that helps prevent fractures) or a placebo for 12 weeks.
All enrolled participants received calcium and vitamin D supplementation, and were randomized to risedronate or matching placebo.
