Research has found that mid-cycle pain (also known as mittelschmerz, German for 'middle pain') occurs just before you ovulate, which would be when you're most fertile.
In the nonsurgical group (110 patients), the major final clinical diagnosis included ovarian cysts (22.7%), acute pancreatitis (10.9%), dyspepsia (8.1%), acute cholecystitis (5.4%), renal colic because of stones (5.4%), and other diagnoses (21.2%): ectopic pregnancy (n = 3), acute cholangitis (n = 1), surgery site abscess (n = 1), gastritis (n = 2), acute hepatitis (n = 1), urinary tract infection (n = 2), biliary colic (n = 4), incarcerated periumbilical hernia (n = 1), varicocele (n = 1), pelvic inflammatory disease (n = 1), and mittelschmerz (n =1) (Table 2 of Supplementary Material), thus leaving 26.3% with nonspecific abdominal pain (NSAP).
The other pathologies that may be encountered are mesenteric lymphadenitis, large or small bowel diverticulitis, right ureteric pathology, and a wide variety of gynecological ailments like ruptured ovarian follicle with midcycle ovulatory bleeding (Mittelschmerz's), ovarian torsion, salpingitis, and ruptured ectopic pregnancy especially in women of child bearing age [9].
The other advantage of laparoscopy is visualization and exclusion of differential diagnosis such as salpingitis, terminal ileitis, typhilitis, ectopic pregnancy, edometriosis, corpus luteal cysts, and tumor infiltrates, paraovarian cysts and mittelschmerz.
