An acute ST-elevation myocardial infarction was diagnosed and a
thrombolytic agent was prescribed, while intravenous calcium replacement had been initiated as part of his resuscitation.
Different in-hospital mortality rates were demonstrated in different studies of subjects with submassive PE to whom the
thrombolytic agent was administered.
Other authors have reported that it may be possible to stabilize the circulation within 10 to 20 minutes when a bolus of a
thrombolytic agent is administered during CPR for suspected PE.
Streptokinase (SK) is a
thrombolytic agent having broad applications in treatment of various diseases like ischemia, Budd-Chiari syndrome, myocardial infarction, brain stroke and myocardial infarction.
If a short-durational and high-dose
thrombolytic agent (100 mg, for 2 hours), such as recombinant tissue plasminogen activator (rt-PA), is applied, the risk of cerebral or noncerebral hemorrhage may increase [1, 2].
Thrombolytic agent is administered with simultaneous localized ultrasound pulsing through the catheter, which is theorized to thin the localized fibrin clot and expose plasminogen receptor sites (3).
Table 2 shows the effectiveness of
thrombolytic agent in each group during hospital stay, at 1-month and 1-year post-AMI follow-up.
Time is of essence and the patient needs to be brought to the hospital quickly for CT scan and administering of the
thrombolytic agent to dissolve the clot or handle the haemorrhage.
As illustrated with this case, fat macroemboli and thromboemboli as causes of acute stroke can be differentiated by CT.This may have potential prognostic and therapeutic implications if a novel
thrombolytic agent or specific interventional angiographic device is to be considered in the future.
Venolysis with local injection of a
thrombolytic agent is preferred by many surgeons.
Other authors support the method of intra--arterial low--dose thrombolysis via the common femoral artery, reasoning that the arterial route delivers the
thrombolytic agent to the arterial capillaries and, subsequently, to the venules.
